care + community health transformation
I partner with non-profit organizations, health systems, and cross-sector initiatives to support collaboration and creative action for improving health equity and community well-being.
My approach is grounded in building authentic relationships, asset-based coaching and capacity building, and creative sense-making and problem solving with and for frontline workers and communities who are marginalized and most impacted. I aim to facilitate learning by doing (early and often) and to lift up on-the-ground expertise and living experiences to uncover new meaning and co-develop meaningful solutions.
With a committment to social change, I am guided by community leaders and wisdom across generations and disciplines -- from health/medicine, urban planning/community development, design/social innovation, to education/youth development, policy/advocacy, and music/arts/culture -- to work toward reimagining systems of care and healing.
10+ years of program design and management experience in health care and community health working in non-profit and public sectors in the Bay Area.
Previous experience: While at Prevention Institute and Center for Care Innovations, I worked in small, but mighty teams to launch and manage various grantmaking initiatives and learning collaboratives for safety net clinics and hospitals to improve health and well-being for BIPOC and low-income communities, including building capabilities in human-centered design, creating innovation hubs to test and spread digital health solutions, and addressing the social determinants of health.
Additional experience: I have also worked for Kaiser Permanente Community Benefit, Pacific Business Group on Health, and City of Richmond + West Contra Costa Unified School District providing strategic planning, research, and program management on a range of regional and national health initiatives. Focus areas include quality improvement in chronic/complex care, healthy eating and active living, and full-service community schools to advance health equity.
Education: Master of Public Health in health policy and management and Master of City Planning in housing, community, and economic development from UC Berkeley

MORE ABOUT ME
Aside from consulting for mission-driven organizations, I also enjoy coaching and mentoring youth and career changers interested in pursuing the health professions and cultivating their creative passions.
My creative passion is advancing social justice through the arts. As a DJ for the past 15+ years, I have performed at various venues and festivals around the country and abroad including SXSW, SF/Oakland Pride, John F. Kennedy Center, National Museum of African American History & Culture, and Manila FlipTop Festival. I’m currently the touring DJ for Oakland-based hip-hop artists and activists - journalist and emcee, Rocky Rivera and four-time Grammy nominee, Alphabet Rockers. Other creative interests I’m currently exploring are public art and site-specific installations/interventions centered on collective care, healing, and health justice.
I was born in Rhode Island to Vietnamese immigrant parents and call Gardena, California home where my family eventually settled. I moved to Berkeley in 2001, as a first-generation college student, and fell in love with the Bay Area ever since. I currently reside in Oakland with my partner, two young sons, and rescue pup. You can find me co-working at local coffee shops in the Glenview neighborhood, live streaming a DJ set on Twitch with Rocky Rivera, or back on the road with Alphabet Rockers to perform our new Grammy-nominated album, The Movement.
